add specifier-instantiator and friends
specifier.c: Add `specifier-instantiator' and related primitives.
lispref/glyphs.texi, lispref/lispref.texi, lispref/specifiers.texi: Use "instantiation" not "instancing". Fix some places where
"specifier" is used to mean "instantiator".
