refactor mc-alloc dependencies

Eliminate the majority of #ifdef MC_ALLOC occurrences through
macros LCRECORD_HEADER, ALLOC_LCRECORD_TYPE, MALLOCED_STORAGE_SIZE,
etc. (defined in lrecord.h).
