menubar-items.el (default-menubar): Reorganize.
Add PROBLEMS to toplevel.
New "More about XEmacs" submenu for NEWS, licensing, etc.
New "Recent History" menu for messages, lossage, etc.
Get rid of ugly and unexpressive ellipses.
